Physical Examination

Students registering for Athletics must have a current physical examination on file in the Activities office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and uploaded prior to the first day of practice/tryouts. Completed physicals MUST be uploaded in “Step 4: Physical Forms” of the online registration form or turned in at the Nurse's Office.

IMPORTANT INFORMATION

To participate on a school-sponsored interscholastic or intramural athletic team or squad, each student whose pre-participation physical examination was completed more than 90 days prior to the first day of official practice shall provide a health history update questionnaire completed and signed by the student’s parent or guardian. Pre-participation physical examinations are good for 365 days.  All students participating in interscholastic athletics must turn in a hard copy of a valid pre-participation physical examination prior to being cleared to participate.

Date of Last Physical Examination - You will need to turn in a hard copy of this completed pre-participation physical form to the Nurse's Office.
